NDIS Support Coordinator

The NDIS Support Coordinator plays a crucial role in assisting participants of the National Disability Insurance Scheme (NDIS) to navigate their NDIS plans, access appropriate services, and achieve their goals. The role involves providing support, guidance, and advocacy to NDIS participants, ensuring they receive the necessary assistance and resources to enhance their quality of life and independence.

Job Description

Key Responsibilities:

  • Conduct initial assessments and develop individualized support plans based on participants' needs, goals, and NDIS funding.
  • Assist participants in understanding their NDIS plans, including budgets, goals, and available supports.
  • Liaise with participants, their families, carers, and relevant stakeholders to coordinate and implement support services.
  • Identify suitable service providers, including allied health professionals, accommodation support services, employment assistance, and community activities, based on participants' preferences and needs.
  • Coordinate appointments, referrals, and ongoing support services to ensure participants' needs are met effectively and efficiently.
  • Provide advocacy and support to participants in navigating the NDIS system, addressing concerns, and advocating for their rights and choices.
  • Monitor participants' progress, review support plans regularly, and make adjustments as necessary to ensure outcomes are achieved.
  • Maintain accurate records, documentation, and reporting in compliance with NDIS guidelines and organizational policies.
  • Stay informed about changes and updates to the NDIS scheme, policies, and procedures, and communicate relevant information to participants and stakeholders.
  • Foster positive relationships with participants, families, service providers, and community organizations to promote collaboration and enhance support networks.

Desired Skills and Experience

  1. Qualifications:
    1. Bachelor's degree or equivalent qualification in social work, disability studies, community services or related field.
    2. Valid's drvier's license and access to a reliable vehicle (where applicable) for travel to meet participant's and attend appointments.
  2. Experience:
    1. Experience working with people with disabilities and/or in the disability sector, preferably in a coordination or support role.
  3. Skills:
    1. Excellent organisational and time - management abilities, with a practive approach to problem solving and decision making.
    2. Strong communication, interpersonal and advocacy skills, with the ability to build rapport and establish trusting relationships.
    3. Knowledge and understanding of the NDIS framework, legislation and processes.
    4. Ability to work independlty and collaboratively within a multidisciplinary team environment.
    5. Proficiency in using computer software and databases for documentation and record keeping purposes.
  4. Personal Attributes:
    1. Empathy, paitence and a non- judgemental attitude towards individuals with disabilities and their families.
    2. Commitment to upholding ethical standards, confidentiality asnd respecting participants rights and dignity.
